http://www.google.com/search?hl=en&q=s...mp;aq=f&oq= <- this line in your log has 0 hits on google , unless you created this yourself I would be willing to bet that this is the cause .

F2 - REG:system.ini: UserInit=C:\WINDOWS\system32\userinit.exe,C:\WINDOWS\system32\wscript.exe C:\WINDOWS\system32\Virusafe.vbs

Fix this line and reboot .

R0 - HKCU\Software\Microsoft\Internet Explorer\Main,Start Page = http://equinox.com/
R1 - HKCU\Software\Microsoft\Internet Explorer\Main,Window Title = Equinox.com

Then fix these two and reboot , see if you have control again .

Yes, you are right the cause was from C:\WINDOWS\system32\Virusafe.vbs, this is a nasty VB script is running as a Hidden & System file in the backgroud. It is also populating the registry with IE start page & window title set to equinox.com. I use the FileAssassin in Malwarebytes to remove the file. But that was not enough, I do a search on the registry for keys having viursafe.vbs and remove a couples of infected key before this is finally over.
